Since 2012, CHAP has been a catalytic philanthropic force in community health, working at the intersection of government and community to build sustainable, integrated, and effective health and care systems.
In recent years, the demand for collaborative, tactical work has necessarily shifted our focus. To meet this need, Envelop Health Consulting has been launched to extend our hands-on approach to a wider community with a commitment to stay until the work takes, and holds, its shape.

Wendy McWeeny, Director, CHAP & Envelop Health
Wendy McWeeny has spent over two decades working at the intersection of philanthropy and public health, bridging the global and local and championing the shift to systems-based approaches to change. Prior to CHAP, Wendy held a senior philanthropic and advisory role at the MCJ Amelior Foundation, the family foundation of Ray Chambers, U.N. and WHO Special Envoy.

Tracy Parris-Benjamin, Senior Advisor, Envelop Health
Tracy Parris-Benjamin is Managing Partner of The Outcomes Architect, an advisory organization working across sectors to reimagine how health is addressed. Prior to this role, Ms. Parris-Benjamin served as Senior Director of Health Equity and Community Health at Horizon Blue Cross Blue Shield of New Jersey.

Nisha Patel, Senior Advisor, Envelop Health
Nisha Patel has led nation-wide public health and policy programs for nearly 25 years. Most recently, Nisha served as Senior Advisor at the Health Resources and Services Administration (HRSA), overseeing issues related to rural health, maternal health, organ transplantation, and behavioral health. She is also the founder of CONNECT Healthcare Solutions.

Tahab Talib, Community Health Project Lead
Tahab Talib is currently leading initiatives to develop sustainable financing pathways for the Community Health Worker (CHW) workforce in Newark, New Jersey. Previously, Tahab served as a Senior Project Lead at Partners In Health US, led the analysis of New Jersey’s first CHW employer landscape survey, and spearheaded the establishment of the New Jersey CHW Association.
